AGREEMENT AND RELEASE OF LIABILITY

This agreement is between the Authentic Pilates Conference “Pilates from Elders” participant and Zip Pilates Studio, Gdynia.

In consideration for Pilates instruction including but not limited to Pilates I make the following statements and promises:

I understand that fitness instruction, as with any sport, could lead to physical injury and I personally assume risk of all injury. I understand that all reasonable efforts will be made to ensure my physical safety.

I understand that the equipment used in the fitness instruction can be dangerous if used improperly and I will follow the strict instructions of the instructor with respect to all use of the equipment.

I agree that I, or my legal representative will not make a claim or initiate any suit against Zip Pilates Studio Gdynia or any of the Pilates Conference Teachers for any injury or damage resulting from my participation in fitness instruction. I agree to hold Zip Pilates Studio Gdynia harmless from any injury I receive from any participation in Pilates.

If I am pregnant or if I become pregnant, or if I have or suffer a serious injury or ailment, I will discontinue training until I receive written consent from my doctor.
